The BePure Health Questionnaire asks you to assess how you have been feeling. It is intended to help raise awareness for adrenal function. For each question, select the answer that best describes your symptoms.
(0) No or Rarely – You have never experienced the symptom or the symptom is familiar to you but perceive it as insignificant (monthly or less).
(1) Occasionally – Symptom comes and goes and is linked in your mind to stress, diet, fatigue or some identifiable trigger.
(4) Often – Symptom occurs 2-3 times per week and/or with a frequency that bothers you enough that you would like to do something about it.
(8) Frequently – Symptom occurs 4 or more times per week and/or you are aware of the symptom every day, or it occurs with regularity on a monthly or cyclical basis.
Some questions require a NO or YES response: 0 = NO, 8 = YES

A score of 15 or lower suggests Adrenal Fatigue isn’t a big issue for you currently. Learning about the signs and symptoms of Adrenal Fatigue, while implementing nourishing nutrition and stress-management strategies will help to ensure this does not become an issue for you in the future.
A score of 16-32 suggests your body and hormone responses to stress and divfestyle factors show signs of Adrenal Fatigue. You need to take adequate action around diet, lifestyle and exercise to restore optimal Adrenal function.
A score of greater than 32 suggests Adrenal Fatigue is a big issue for your health. A score this high should not be ignored. Learn about nourishing diet, nutritional, lifestyle and stress management strategies to begin Adrenal recovery should be a priority for you. If left untreated your health could significantly suffer.
This questionnaire has been developed by the BePure nutritional team as a tool to raise awareness for adrenal function. It is not designed to diagnose or treat adrenal sufficiency.
